ELIGIBILITY: Contest is only open to individuals who are eighteen (18) years of age or older as of the date of submission, who are legal residents of Arizona, and have not been convicted of a violent offense, felony, crime of moral turpitude, or misdemeanor, other than minor traffic ticket(s) or parking violation(s). Illicit drug use is highly discouraged and may cause ineligibility if there is a high probability it will negatively interfere with dental treatment.  Employees of participating offices, their affiliates, subsidiaries, advertising, promotion and internet vendors and members of their households and immediate family members are not eligible to enter or win this Contest ("Promotion Parties").

WINNER: Between the designated application dates set by San Tan Smiles, one (1) winner will be chosen from the eligible finalists by that office, in its sole discretion based upon the same Judging Criteria. The winner will be notified by e-mail and/or telephone on or about the designated announcement date. The office will notify the winner, pending verification of winner's eligibility. In order to claim the Prize, the winner must comply with all of the following: 1) respond to the notification, complete and return all of the foregoing documentation within three (3) business days from the date of the prize notification, 2) execute and return any and all forms necessary to claim the Prize, which includes but is not limited to an Affidavit of Eligibility, which includes a liability release that releases the participating office from all liability in connection with the Contest, prize, and any dental treatment, and a publicity release (where permitted by law) and 3) refrain from any illicit drug use prior to or during dental treatment. If the winner fails to comply with any of the above conditions, the prize may be forfeited.  Return of any prize notification as undeliverable may result in disqualification.

Chosen candidates are required to sign a media release form giving San Tan Smiles, its affiliates, successors and assignees, and any person acting under their permission and authority the unqualified right, privilege and permission to use their name as well as reproduce in any manner or form, publish, republish and circulate photographs, videos, slides, transparencies and recordings of their voices or any other images as may be necessary, with or without my given name, or with a fictitious name for advertising, education, or any other lawful purpose.
